Law:: Marine Pollution Control Act Enforcement Rules

Those who issue notifications pursuant to Article 19, Paragraph 1, Article 22, Paragraph 1, Article 26, Paragraph 1, Article 27, Paragraph 1 applied to Article 26, Paragraph 1 or Article 33, Paragraph 1 of this Act shall include the following items.1. The name, title, organization and premises of the reporting person2. The source and cause of the pollution3. Time and location or latitude and longitude of the incident4. Pollutant types and characteristics5. Degree and quantity of pollution and measures already adopted6. Meteorological conditions and their possible impact on pollution7. Phone, fax and other contact methods for emergency notification8. Other items designated by the central competent authorityNotified agencies shall maintain records of the content of notifications in the foregoing paragraph.

中文原文海洋污染防治法施行細則第15條

依本法第十九條第一項、第二十二條第一項、第二十六條第一項、第二十七條第一項準用第二十六條第一項或第三十三條第一項規定所為之通知，其內容包括下列事項：
一、報告人姓名、職稱、單位、場所。
二、污染發生來源、原因。
三、發生事故時間、位置或經緯度。
四、污染物種類及特性。
五、污染程度、數量及已採取措施。
六、氣象狀況及可能之污染影響。
七、緊急通知電話、傳真或其他聯絡方式。
八、其他經中央主管機關指定之事項。
受通知機關就前項通知內容應作成紀錄。
