The Regulation Governing the Handling of Investigation Procedures for Civil and Public Aircraft Occurrence Article 4

In the event of a major aviation occurrence or a suspected major aviation occurrence in accordance with Article 6 of the Act, the owner and operator of the aircraft, as well as the air traffic control agency (or institution), shall promptly notify the TTSB duty officer by telephone and submit a completed Major Aviation Occurrence Report Form to the TTSB by facsimile or e-mail, within the time limit specified in Article 9 of the Act.
