Enforcement Rules of the Nuclear Materials and Radioactive Waste Management Act Article 9
To apply for renewal of an operating license according to Paragraph 3 of Article 9 of this Act, the applicant shall submit the following documents; where necessary, the competent authority may require the applicant to bring forward a decommissioning plan:1.Safety analysis report of the latest edition2.Safety evaluation report for renewal of license.3.Photocopy of the storage permit(s) or entrusted treatment contract(s) issued by domestic and/or foreign radioactive waste final disposal facility.

依本法第九條第三項規定向主管機關申請換發運轉執照者，應填具申請書，並檢附下列資料，必要時，主管機關得要求其併提出除役規劃報告：
一、最新版之安全分析報告。
二、換照安全評估報告三、國內、外放射性廢棄物最終處置設施之貯存許可或代處理契約影本。
