Enforcement Rules of the Nuclear Materials and Radioactive Waste Management Act Article 15
To apply for nuclear source material or nuclear fuel production facility operator certificate, the applicant shall submit a letter of application enclosed with the following documents to the competent authority:1.Photostat copy of education certificates.2.Training certificates.3.Field practice certificate.The validity period of the above-mentioned training certificate is 3 years. A letter of application and on-job retraining certificate shall be submitted to the competent authority 30 days prior to expiration, to apply for renewal of the certificate.

申請核子原料、核子燃料生產設施運轉人員合格證明書者，應填具申請書，並檢附下列證明文件，向主管機關為之：
一、學歷證件影本。
二、訓練合格證明。
三、現場實習證明。
前項合格證明書有效期間三年，期滿三十日前，應填具換發申請書及再訓練合格證明，向主管機關申請換發。
