Administrative Regulations for Major Dispute Mediation of Telecommunications Enterprises Article 12
The panel shall prepare the mediation decision into a certificate, which shall record the following items and be signed by the parties and the mediator:1.The name of the telecommunication enterprises, offices or business offices, the name, date of birth, residence and ID Card number of the administrator or representative.2.Where there is a legal representative or a proxy is appointed, the name, residence and ID Card Number of the proxy.3.The names of the attending mediator and the assisting persons present.4.Cause for the mediation.5.The subject matter of the mediation.6.Location of the mediation.7.The date of the mediation.The competent authority shall serve the mediation certificate tothe parties within 15 days after the certificate had been made.

調處會應將調處結果作成調處證明書，記載下列事項，並由當事人及調處委員簽名：

二、有法定代理人或委任代理人者，其姓名、出生年月日、住居所及身分證字號。
三、出席調處委員及列席協同調處人之姓名。
四、調處事由。
五、調處之內容。
六、調處之場所。
七、調處之年月日。
主管機關應於調處證明書作成後十五日內，送達當事人。
