Regulations for Approving and Supervising Cultural Public-interest Trusts Article 22

The interested party or the prosecutor, when requesting the removal of the trust supervisor's office in accordance with Article 58 and Article 76 of the Law, shall submit two copies of each of the following documents and apply with the competent authority:1. an application letter;2. a document stating the reason of removing the trust supervisor's office; and3. an opinion with respect to the choice of a new trust supervisor.

利害關係人或檢察官依本法第五十八條及第七十六條規定，申請解任信託監察人者，應檢具下列文件一式二份向主管機關提出：
一、申請書。
二、申請解任理由書。
三、有關新信託監察人選任之意見。
