Air Pollution Control Act Enforcement Rules Article 32
The written report to be submitted within 15 days of the malfunction designated in Article 89, subparagraph 3 of this Act shall contain the following content.1. Name and location of the equipment2. Reason for occurrence and repair methods3. Pollution control measures adopted during the period of malfunction and estimated air pollutant emissions quantities4. Methods used to prevent the recurrence of similar malfunctions5. Other items designated by the competent authority

本法第八十九條第三款所定故障發生後十五日內提出之書面報告，其內容應包括：
一、設備名稱及位置。
二、發生原因及修護方法。
三、故障期間所採取污染防制措施及估計空氣污染物排放量。
四、防止未來同類故障再發生之方法。
五、其他經直轄市、縣（市）主管機關指定之項目。
