Regulations for Assisted Reproduction Institution Permit Article 16
Where a public interest society for the first time applies for permission to establish a sperm bank as provided in Paragraph 2 of Article 6 of this Act, it shall submit the following required documents:1. Society or foundation registration certificate.2. Personnel roster and documents verifying qualifications as prescribed in the preceding two Articles.3. Relevant facilities and equipment specified in attached Table 5.4. 公益法人首次依本法第六條第二項申請設立精子保存庫時，應檢附下列文件：

四、冷凍作業手冊、實驗室品質管制作業手冊、冷凍作業品質管制及儀器設備測試紀錄等文件。
