Enforcement Rules of the Physicians Act Article 19
To report the discontinuation or suspension of practice in accordance with Article 10, Paragraph 1 of the Act, physicians shall fill out the designated application form, attach their practice license and relevant supporting documents, and submit them to the original issuing authority for processing:1. Discontinuation: Practice registration and practice license shall be revoked2. Suspension: Practice license shall be returned following recording of the period of and reasons for suspension
中文原文 醫師法施行細則第19條
醫師歇業、停業，依本法第十條第一項規定報請備查時，應填具申請書，並檢具執業執照及有關文件，送由原發執業執照機關依下列規定辦理：
一、歇業：註銷其執業登記及執業執照。
二、停業：登記其停業日期及理由後，發還其執業執照。
