Where the practice license of a nutrition counseling institution is destroyed or lost, the institution shall submit a completed application form together with the required processing fee to the original license issuing authority to process the reissuance of a new certificate.Where the practice license is damaged, the institution shall submit a completed application form and the original practice license together with the required processing fee to the original license issuing authority to process the license replacement.

營養諮詢機構開業執照滅失或遺失者，應填具申請書，並檢附開業執照費，向原發開業執照機關申請補發。
開業執照損壞者，應填具申請書，並檢附開業執照費，連同原開業執照，向原發開業執照機關申請換發。
