The Enforcement Rules for the Forestry Act Article 17
An applicant for the classification or declassification of a conservation forest in accordance with Article 26 of this Act shall complete a relevant application form, attached with a location map, providing the following information:1. The name, location and area of the forest to be classified or declassified;2. The grounds for classification or declassification;3. The name and address of the applicant. For juristic person or organization, the name and address, and its representative and the names of responsible person must be provided.

依本法第二十六條規定申請保安林編入或解除，應填具申請書並檢附位置圖，載明下列事項：
