Occupational Safety and Health Education and Training Regulations Article 20-4
The application for assessment referred to in Paragraph 1 of the preceding article shall be submitted to the central competent authority together with the following documents:1.Self-evaluation form;2.Qualification documents that meet the requirements under Paragraph 1 of the preceding article;3.Other documents required by the central competent authority.Where the documents referred to in the preceding paragraph are found incomplete, the central competent authority may notify the applicant to make correction within specific time limit. The application will not be accepted if the applicant fails to make corrections within the specific time limit, or makes corrections defectively.

三、其他經中央主管機關規定之文件。
