Occupational Safety and Health Education and Training Regulations Article 20-1
The application for approval referred to in Paragraph 2 of the preceding article shall be submitted to the central competent authority together with the following documents:1.Application form;2.Photocopy of the establishment certificate of vocational training institution;3.Safety and Health promotion achievement certificate;4.Other documents required by the central competent authority.Where the documents referred to in the preceding paragraph are found incomplete, the central competent authority may notify the applicant to make correction within specific time limit. The application will not be accepted if the applicant fails to make corrections within the specific time limit, or makes corrections defectively.

申請前條第二項之認可者，應檢附下列文件，向中央主管機關提出：
一、申請書。
二、職業訓練機構設立證書影本。
三、安全衛生推廣績效證明文件。
四、其他經中央主管機關規定之文件。
前項文件未備齊者，中央主管機關得通知其限期補正；屆期未補正或補正未完全者，不予受理。
