Enforcement Rules of the Labor Pension Act Article 37
Workers claiming for labor pension in accordance with Paragraph 1 of Article 28 of the Act shall fill out the Labor Pension Application Form.Workers claiming for labor pension in accordance with Paragraph 1 of Article 24-2 of the Act shall fill out the Application Form for Early Claim of Labor Pension; those claiming pursuant to Subparagraph 3 of Paragraph 1 of the same Article shall additionally submit photocopies of the front and back of the severe or higher-level disability certificate.Applicants referred to in the preceding two paragraphs who do not have household registration in the Republic of China shall additionally submit relevant identification documents.

勞工依本條例第二十八條第一項規定請領勞工退休金時，應填具勞工退休金申請書。
勞工依本條例第二十四條之二第一項規定請領勞工退休金時，應填具提前請領勞工退休金申請書；依該條第一項第三款規定請領者，並應檢附重度以上身心障礙證明正背面影本。
前二項請領人未於國內設有戶籍者，應另檢附身分證明相關文件。
