Criminal Compensation Act Article 10

(Claims for compensation)A claim for compensation shall specify the following information in writing and shall be submitted to judicial agencies which have jurisdiction to adjudicate the claim:1. the claimant's name, gender, age, domicile or residence.2. the legal representative's name, gender, age, domicile or residence.3. the subject matter of the claim.4. facts and reasons of the claim; original copy or proof of the prosecutor's ruling of "not to prosecute", withdrawal of prosecution, adjudication on which the compensation claim is based should be attached.5. the agency with jurisdiction.6. year, month, and date.
中文原文 刑事補償法第10條
補償之請求，應以書狀記載下列事項，向管轄機關提出之：
一、補償請求人姓名、性別、年齡、住所或居所。
二、有代理人者，其姓名、性別、年齡、住所或居所。
三、請求補償之標的。
如請求為分期支付，其分期方式及金額。
四、事實及理由，並應附具請求補償所憑之不起訴處分書、撤回起訴書，或裁判書之正本或其他相關之證明文件。
五、管轄機關。
六、年、月、日。
