The application for the confidentiality preservation order shall include the following information in writing:1. Persons to receive the confidentiality preservation order2. The trade secrets to be protected by the order3. Facts consistent with the conditions listed in all the subparagraphs in the first paragraph of the preceding articleThe description of the trade secrets in Subparagraph 2 of the preceding paragraph can be disclosed by indirect citation.

秘密保持命令之聲請，應以書狀記載下列各款事項：
一、應受秘密保持命令之人。
二、應受命令保護之營業秘密。
三、符合前條第一項各款所列事由之事實。
