Constitutional Court Procedure Act Article 68
(Contents of Pleadings Concerning Impeachment) The Legislative Yuan may lodge a petition with the Constitutional Court for a judgment upholding the impeachment against the President or the Vice President pursuant to Article 4, Paragraph 7 of the Additional Articles of the Constitution.Petitions under the preceding paragraph shall be submitted in writing and include the particulars below:(1) the name of the petitioning authority, the location thereof and its representative;(2) the name, the occupation, and the domicile or the residence of the advocate, where applicable;(3) the name, and the domicile or the residence of the impeached;(4) the proceedings leading to the adoption of the impeachment resolution;(5) the underlying facts and evidence supporting the impeachment and the reasons to remove the impeached from office; and (6) the titles of the annexes and the number thereof.

立法院得依憲法增修條文第四條第七項規定，就總統、副總統提出彈劾案聲請憲法法庭為宣告彈劾成立之判決。
前項聲請，應以聲請書記載下列事項：
一、聲請機關名稱、代表人及機關所在地。
二、有訴訟代理人者，其姓名、職業、住所或居所。
三、被彈劾人之姓名、住所或居所。
四、彈劾案決議作成之程序。
五、彈劾之原因事實、證據及應予解職之理由。
六、關係文書之名稱及件數。
前項聲請，應以聲請書記載下列事項：

二、有訴訟代理人者，其姓名、職業、住所或居所。
三、被彈劾人之姓名、住所或居所。
