Standards Governing the Establishment of Commercial Banks Article 13
The bank whose establishment is approved by the competent authority shall apply to the competent authority for the business license by attaching three copies of each of the following documents within three months after incorporation registration:1. application form for business license2. certificate of incorporation registration3. certificate for verification of capital4. articles of incorporation of the bank5. the minutes of the founders’ meeting6. the list of shareholders and the minutes of shareholders’ meeting7. the list of directors and the minutes of board of directors’ meeting8. the list of managing directors and the minutes of board of managing directors’ meeting9. the list of supervisors and the minutes of supervisors’ meeting10. the list of managers11. bank rules and business operation procedures12. the certification of qualifications for directors, supervisors and managers13. the simulated business operation records lasted for more than two weeksBefore the expiry of the period specified in the preceding paragraph, the extension application may be granted if there is a legitimate reason. Such extension application shall not exceed three months, and is only limited to one time. The competent authority may revoke the permits for those whose extension applications are not approved.

設立銀行者，應於辦妥公司設立登記後三個月內，檢同下列書件各三份，向主管機關申請核發營業執照：
一、營業執照申請書。
二、公司登記證件。
三、驗資證明書。
四、銀行章程。
五、創立會會議紀錄。
六、股東名冊及股東會會議紀錄。
七、董事名冊及董事會會議紀錄。
八、常務董事名冊及常務董事會會議紀錄。
九、監察人名冊及監察人會議紀錄。
十、經理人名冊。
十一、銀行章則及業務流程。
十二、董事、監察人及經理人之資格證明文件。
十三、兩週以上之模擬營業操作紀錄。
前項規定期限屆滿前，如有正當理由，得申請延長，延展期限不得超過三個月，並以一次為限，未經核准延展者，主管機關得廢止其許可。
