Management Regulations for Reuse of Industrial Waste in Science Parks Article 7
Application for general case permission: fifteen (15) copies of the application form shall be submitted by the reuse organizations to the Bureau.The application form mentioned in the preceding paragraph shall comprise of:1. Basic information of the reuse organization2. The scheme of reusing operation3. Other documents as designated by the BureauThe scheme of reusing operation mentioned in Subparagraph 2 of the preceding paragraph shall comprise of:1. Basic information of industrial waste2. Plan of clearance3. The scheme of reusing, which contains monthly statistical data of quantity that is accepted, reused and stored over twelve (12) months for industrial waste of the same industrial process4. Plan of pollution prevention5. The quality control and plan for sale of reused products, which contains monthly statistical data of sale6. Irregular manipulation plan7. Emergency management plan8. Evidence issued by the municipal or county (city) authority(ies) to prove that within one year before the date of the application, the applicant has not been requested for correction within a limited period of time and failed to complete improvements within a limited period of time because of violation of environmental protection laws, and has not been penalized with a fine on a daily basis (based on the number of time(s)), penalized with suspension of work, suspension of business, termination of business, revocation or cancellation of permits (documents), or other deprivation or extermination of certain qualifications or rights, or is accused because of involvement in criminal liability9. The product continuous audit plan of the reuse organizationThe monthly statistical data mentioned in Subparagraphs 3 and 5 and the evidence mentioned in Subparagraph 8 of the preceding paragraph are restricted to the same corporation. And if they are written in foreign language, the translation in Chinese that is verified by an institution functioning abroad or an organization authorized by the Ministry of Foreign Affairs shall be submitted.

通案再利用許可之申請，由再利用機構檢具再利用許可申請文件一式十五份，向管理局為之。
前項申請文件，應包括：
一、再利用機構基本資料。
二、再利用運作計畫書。
三、其他經管理局指定文件。
前項第二款再利用運作計畫書內容，應載明：
一、事業廢棄物基本資料。
二、清除計畫。
三、相同產業製程事業廢棄物，具有十二個月以上之收受量、再利用量及暫存量月統計資料之再利用計畫。
四、污染防治計畫。
五、具有產品產銷月統計資料之產品品管及銷售計畫。
六、異常運作處理計畫。
七、緊急應變計畫。
八、由所在地之直轄市或縣（市）主管機關開具申請前一年內未曾因違反環保法令受限期改善而屆期仍未完成改善、按日（次）處罰鍰、停工、停業、勒令歇業、撤銷、廢止許可證（文件）或其他剝奪或消滅一定資格或權利之處分，或涉及刑事責任經起訴之證明文件。
九、再利用機構對產品之追蹤稽核計畫。
前項第三款、第五款之月統計資料及第八款之證明文件，以同一法人為限；如為外文者，應檢附駐外單位或外交部授權機關驗證之中譯本。
