Stationary Pollution Source Installation, Operating and Fuel Use Permit Management Regulations Amended Clauses Article 30
Public or private premises that apply for the extension of a permit in accordance with Article 30 of this Act shall fill out an application form and submit below documents to the reviewing agency:I.Those that apply for the extension of an installation permit shall submit an installation project progress change explanation form.II.Those that apply for the extension of an operating permit shall submit the most recent testing report from within the last year or other verification documents sufficient to demonstrate compliance with the relevant regulations of this Act. If, however, public or private premises cannot submit the most recent testing report from within the last year due to suspension of work or business, such public or private premises may apply for extension using alternative verification documents after reporting to and obtaining the consent of the reviewing agency.III.Other documents designated by the central competent authority.

公私場所依本法第三十條規定申請許可證展延者，應填具申請表，並檢具下列文件，向審核機關為之：
一、申請展延設置許可證者，應檢具設置工程進度變更說明表。
二、申請展延操作許可證或燃料使用許可證者，應檢具一年內最近一次之檢測報告，或其他足以說明符合本法相關規定之證明文件。
但因停工（業）無法檢具一年內最近一次檢測報告者，得報經審核機關同意，以替代證明文件辦理。
三、其他經中央主管機關指定之文件。
一、申請展延設置許可證者，應檢具設置工程進度變更說明表。
二、申請展延操作許可證或燃料使用許可證者，應檢具一年內最近一次之檢測報告，或其他足以說明符合本法相關規定之證明文件。
但因停工（業）無法檢具一年內最近一次檢測報告者，得報經審核機關同意，以替代證明文件辦理。
三、其他經中央主管機關指定之文件。
