Stationary Pollution Source Installation, Operating and Fuel Use Permit Management Regulations Amended Clauses Article 25
The following items shall be recorded for the stationary pollution source fuel usage permit:I.The validity period and certificate number of the fuel uesage permit.II.Basic Information:(I)The name and address of the public of private premises.(II)The name, identification document number and address of the statutory responsible person of the public or private premises.III.Fuel usage permit contents:(I)The name of the stationary pollution source, flowcharts of manufacturing processes, types, compositions and usage quantities of fuel connected with air pollutant emissions and operating schedules.(II)The name and model of air pollution control methods and facilities.(III)Regulations about fuel use records.(IV)Other permission items designated by the central competent authority.The records of Item (III), Subparagraph III of the previous paragraph shall be kept for six years for future reference.Electronic records shall be established for the information kept for future reference. If the electronic records cannot be implemented, with the consent of the competent authority, paper records may be used instead.

固定污染源之燃料使用許可證應記載事項如下：
一、燃料使用許可證之有效期間及證書字號。
二、基本資料：
（一）公私場所名稱及地址。
（二）公私場所負責人姓名、身分證明文件字號及住址。
三、燃料使用許可內容：
（一）固定污染源名稱、製程流程圖說、與空氣污染物排放有關之燃料種類、成分、用量及其操作期程。
（二）空氣污染防制方法及設施之名稱及型式。
（三）燃料使用紀錄之規定。
（四）其他經中央主管機關指定之許可事項。
前項第三款第三目所列之紀錄，應保存六年備查。
前項備查資料應建立電子化紀錄，未能執行電子化紀錄者，經主管機關同意後，得以紙本紀錄代之。
