The fees that will be charged to the applicant requesting the Insurer to provide personal data are as follows:1. Simplified Format: NT$300 per request.2. Expense Format: NT$600 per request.3. Medical Format: NT$1,500 per request.The maximum amount for the preceding paragraph shall not exceed NT$1,500 per request.

保險人提供下列格式之個人資料，其收費如下：

二、費用格式：每人次收費新臺幣六百元。
三、醫令格式：每人次收費新臺幣一千五百元。
前項收費總額，每人次以新臺幣一千五百元為上限。
