Regulations Governing the Trace and Track System for Medicinal Products Article 5
Distributors who engage in business of pharmaceutical wholesaling should establish and declare the following information for their medicinal products:1. Information regarding product supplier:(1) Name, address, contact person and telephone number of supplier.(2) Name of the medicinal product and approved number on the medicinal product license.(3) Batch number.(4) Quantity.(5) Manufacture date.(6) Expiry date or shelf life.(7) Delivery date.2. Information regarding flow of medicinal product:(1) Name, address, contact person and telephone number of receptor.(2) Name of the medicinal product and product license number.(3) Batch number.(4) Quantity.(5) Manufacture date.(6) Expiry date or shelf life.(7) Delivery date.Distributors who engage in business of pharmaceutical wholesaling should submit all the trace and track information of previous month in electronic form by the 10th of each month.

藥品許可證所有人以外之販賣業者從事製劑批發業務時，應建立及申報下列資訊：
一、製劑供應商資訊：
（一）供應商之名稱、地址、聯絡人及電話。
（二）藥品許可證所載品名及核准字號。
（三）批號。
（四）數量。
（五）製造日期。
（六）有效期間或保存期限。
（七）收貨日期。
二、製劑流向資訊：
（一）供應對象之名稱、地址、聯絡人及電話。
（二）藥品許可證所載品名及核准字號。
（三）批號。
（四）數量。
（五）製造日期。
（六）有效期間或保存期限。
（七）交貨日期。
前項販賣業者，應於每月十日前將上月份藥品追溯或追蹤資訊，以電子方式申報至追溯或追蹤申報系統。
