Regulations Governing Quarantine at Ports Article 27
The Ship Declaration of Health submitted by inbound ships from foreign countries (boundaries) when applying for port entry in accordance with the regulations specified in Article 10 shall be completed and signed by the shipmaster. The ship physician, if on board, shall also countersign the Declaration.

自國（境）外進港之船舶，依第十條規定申辦入港手續時，其檢附之船舶健康聲明書，應由船長填寫並簽名；該船舶置有船醫者，並應經船醫副署。
