Regulations Governing Subsidies for Injuries, Illnesses or Deaths due to Performing Control Measures against Category V Communicable Diseases Article 7
Claimants, when applying for subsidies in accordance with Paragraph 1 of Article 2 herein, shall fill in an application form and submit it together with the following documents to the central competent authority; the application may also be approved and then forwarded to the central competent authority by the claimant’s service unit:1. Injuries or illnesses due to infection with Category V Communicable Diseases:1) Certificate of diagnosis of a Category V Communicable Disease issued by a hospital;2) A document issued by the relevant organization evidencing infection with a Category V Communicable Disease in the course of performing control measures;3) A report of the competent authority verifying infection with a Category V Communicable Disease; and4) A photocopy of both sides of the claimant’s national identification card.2. Physical or mental disabilities due to infection with Category V Communicable Diseases:1) All documents specified in the items of the preceding Subparagraph; and2) A certificate of physical or mental disability.3. Deaths due to infection with Category V Communicable Diseases:1) All documents specified in the items of Subparagraph 1;2) A document issued by a hospital evidencing the cause of death being infection with a Category V Communicable Disease;3) A household registration transcript removing the deceased; and4) A complete household registration transcript (that shows the relationship between the deceased and the surviving members of the family).4. Education costs of children:1) All documents specified in the items of Subparagraph 1;2) A certificate of physical or mental disability, or a document issued by a hospital evidencing the cause of death being infection with a Category V Communicable Disease; and3) A photocopy of the child’s student identification and receipts of fee payments.

（二）相關單位出具係因執行防治工作致感染第五類傳染病之證明文件。
（三）主管機關確認罹患第五類傳染病報告。
（四）國民身分證正反面影本。
二、感染第五類傳染病致身心障礙：
（一）前款各目規定文件。
（二）身心障礙證明。
三、感染第五類傳染病致死亡：
（一）第一款各目規定文件。
（二）醫院出具死亡原因為感染第五類傳染病之證明文件。
（三）死亡者除戶戶籍謄本。
（四）全戶戶籍謄本（應能檢視與死亡者之遺族關係）。
四、子女教育費用：
（一）第一款各目規定文件。
（二）身心障礙證明或醫院出具死亡原因為感染第五類傳染病之證明文件。
（三）學生證影本及繳費單據。
