Enforcement Rules of the Physicians Act Article 16

In the event of a lost or damaged physician, doctor of Chinese medicine, or dentist license, the applicant shall fill out the designated application form, pay the prescribed reissuance fee, and, in the case of a damaged license, submit the original license to the central competent authority for reissuance or replacement.The application process referred to in the preceding paragraph may be conducted electronically through a system established by the central competent authority.
