Psychologists Act Article 15
In practicing his/her profession, a psychologist shall make records and specify the following:1. The client ’s name, gender, date of birth, ID number, and address.2. The practice of clinical or counseling psychological practice and the date thereof.3. Other matters that shall be specified according to the regulations.

心理師執行業務時，應製作紀錄，並載明下列事項：
一、個案當事人之姓名、性別、出生年月日、國民身分證統一編號及地址。
二、執行臨床心理或諮商心理業務之情形及日期。
三、其他依規定應載明之事項。
