National Health Insurance Act Article 18
The insurance premium payable by the insured in Categories 1 to 3 and his/her dependents shall be calculated according to the insured payroll-related amount and the insurance premium rate of the insured. The insurance premium rate shall be set at a maximum of 6 percent.The insurance premium payable by the dependents articulated in the preceding paragraph shall be paid by the insured. When the number of dependents exceeds three, the insurance premium shall be calculated on the basis of only three dependents.

第一類至第三類被保險人及其眷屬之保險費，依被保險人之投保金額及保險費率計算之；保險費率，以百分之六為上限。
前項眷屬之保險費，由被保險人繳納；超過三口者，以三口計。
