Veterinarian Act Article 12
12.1 A practicing veterinarian shall make entries about the diagnosis, treatment and/or inspection in the corresponding medical record or inspection record when conducting diagnosis, treatment, or inspection.12.2 The medical record in Section 12.1 （paragraph 1 of Article12） shall contain:（1）The name and address of the animal owner,（2）The species and weight of the animal,（3）For each visit –date of diagnosis and/or treatment, physical conditions, diagnostic results as well as prevention, medication used and treatment performed.（4）If a controlled substance is administered – the drug name, dosage and application method.

執業之獸醫師施行診斷、治療或檢驗時，應將診斷、治療或檢驗事項分別記入診療紀錄或檢驗紀錄。
前項診療紀錄，應記載下列事項：
一、飼主之姓名及地址。
二、動物之種類名稱、體重。
三、各次之診療日期、發病情形、診斷結果及預防、用藥與治療情形。
四、使用管制藥品者，其藥品品名、藥量及用法。
