Veterinary Drugs Control Act Article 12-1
A veterinary drug license shall contain the following information:1. License number;2. Name of the veterinary drug;3. Names and addresses of the manufacturer and/or importer;4. Name and address of the person in charge;5. Name and address of the manufacturing factory;6. Pharmaceutical form and packaging;7. Name and the amount of each active ingredient;8. Indications; and9. Other information required by the central competent authority.

動物用藥品許可證應記載事項如下：
一、許可證字號。
二、動物用藥品名稱。
三、製造業者與輸入業者名稱及地址。
四、負責人姓名及地址。
五、製造工廠名稱及地址。
六、動物用藥品劑型及包裝。
七、有效成分及含量。
八、效能（適應症）。
九、其他經中央主管機關指定應記載事項。
