To apply for the review of Category B Workplace by labor inspection agency, enterprises should complete the application form (Form 2) and attach three copies of the following documents：1. Basic information on safety and health management (Attachment 1)2. Process safety evaluation report (Attachment 2)3. Process change safety plan (Attachment 3)4. Emergency response plan (Attachment 4)5. Audit management program (Attachment 5)

事業單位向檢查機構申請審查及檢查乙類工作場所，應填具申請書（如格式二），並檢附下列資料各三份：
一、安全衛生管理基本資料，如附件一。
二、製程安全評估報告書，如附件二。
三、製程修改安全計畫，如附件三。
四、緊急應變計畫，如附件四。
五、稽核管理計畫，如附件五。
