Enforcement Rules of the Employment Insurance Act Article 16-1
An insured person applying for parental leave allowance according to Item 4 of Paragraph a pf Article 11 of this Act shall prepare the following documents:1.The Parental Leave Allowance Application Form and Benefits Receipt.2.A duplicate of the household registration of the insured and the dependents, or other sufficient proof of identity.3.Proof of parental leave without pay.4.A duplicate of the insured person's personal bank passbook for a domestic bank.5. Foreign professionals, foreign specialist professionals, or foreign senior professionals shall additionally enclose a copy of a valid permanent residency document.

被保險人依本法第十一條第一項第四款規定請領育嬰留職停薪津貼者，應備具下列書件：
一、育嬰留職停薪津貼申請書及給付收據。
二、被保險人及子女之戶口名簿影本或其他身分證明文件影本。
三、育嬰留職停薪證明。
四、被保險人本人名義之國內金融機構存摺影本。
五、外國專業人才、外國特定專業人才或外國高級專業人才，另檢附有效之永久居留證明文件影本。

三、育嬰留職停薪證明。
