Enforcement Rules of the Electronic Signatures Act Article 13
When a certification authority files an application to the competent authority for modification of its certification practice statement, in accordance with Paragraph 1, Article 12 of the Act, the application form and the following documents shall be submitted:1. The modified certification practice statement and the checklist of the required information for the certification practice statement thereof;2. A comparison table of content variation; and3. Other documents required by the competent authority.The format of the application form and documents stipulated in the preceding paragraph shall be prescribed by the competent authority.

憑證機構依本法第十二條第一項規定，向主管機關申請變更憑證實務作業基準者，應檢具申請書及下列文件：
一、變更後之憑證實務作業基準及其應載明事項檢核對照表。
二、變更內容對照表。
三、其他經主管機關指定之文件。
前項申請書及文件之格式，由主管機關定之。
