Identification Regulations for Operational Headquarters Article 4
A company filing an application for determination of Operation Headquarters pursuant to Items 1 to 5 of Paragraph 1 of Article 2 shall submit the following supporting documents and have the documents signed by a CPA:1. Education qualifications, lists of labor and national health insurance or statistical forms of formal employees;2. Profit-seeking income tax return forms;3. Overview of domestic and international affiliated enterprises, affiliates abroad and the relevant documents showing overseas branch offices being registered in accordance with the law.4. Annual reports and consolidated financial statements.

公司依第二條第一項第一款至第五款規定申請認定營運總部，應檢具下列證明文件，並經會計師簽核：
一、正式員工之學歷、勞健保之清冊或統計表單。
二、營利事業所得稅結算申報書。
三、國內外關係企業組織圖及國外關係企業或國外分支機構依法辦理登相關文件。
四、年報或合併財務報表。
