The Regulations Governing Disbursement of Compensations to Prisoners and Defendants Article 4
Applications for medical compensations shall be made to the original execution authority along with the following supporting documents, and shall be paid under the non-working fund of the authority:1. A diagnosis certificate issued by a medical institution.2. Original medical receipts.Applications for disability or death compensations shall be made by the authority to the Work Fund Management Committee of the correction authority of Ministry of Justice for the Committee’s review and approval, along with the following supporting documents:1. Application for Compensation Report Form.2. A disability level certificate or death certificate issued by a medical institution and the household registration certificate containing the date of death.The Application for Compensation Report Form in the preceding paragraph shall state the cause of disability or death and the handling situation.

醫療補償金應檢具下列文件向原執行機關申請，由機關作業基金業務外費用項下支付：
一、醫療機構出具之診斷書。
二、醫療相關收據正本。
申請失能或死亡補償金應由機關檢具下列文件報請法務部矯正機關作業基金管理會核准後發給：
一、申請補償金報告表。
二、醫療機構出具之失能等級證明書或死亡證明書及載有死亡日期之戶籍證明資料。
前項申請報告表應敘明失能或死亡原因及處理情形。
一、醫療機構出具之診斷書。
二、醫療相關收據正本。
申請失能或死亡補償金應由機關檢具下列文件報請法務部矯正機關作業基金管理會核准後發給：
一、申請補償金報告表。
二、醫療機構出具之失能等級證明書或死亡證明書及載有死亡日期之戶籍證明資料。
前項申請報告表應敘明失能或死亡原因及處理情形。
