Enforcement Rules of the Administrative Execution Act Article 40
When a person or his/her agent demands the reimbursement for special loss in accordance to the Article 41 of this Act, they shall send the following information in writing with his/her signature or seal to the enforcement authority:1. The name, sex/gender, date of birth, identification number, occupation and residence or office of the person demanding the reimbursement.2. If there is an agent, his/her name, sex/gender, date of birth, identification number, occupation and residence or office of the agent.3. The reason, facts and evidence for demanding.4. The demanding amount.5. The enforcement authority.6. Year, month, date.

依本法第四十一條請求，特別損失之補償時，請求人或其代理人應以書面載明下列事項，並於簽名或蓋章後，向執行機關提出：
一、請求人之姓名、性別、出生年月日、國民身分證統一編號、職業及住居所。
二、有代理人者，其姓名、性別、出生年月日、國民身分證統一編號、職業及住居所或事務所。
三、請求補償之原因事實、理由及證據。
四、請求補償之金額。
五、執行機關。
六、年、月、日。
