中文原文犯罪被害人保護法施行細則第38條

審議會及覆審會依本法第六十四條及第六十五條所為之決定，應作成決定書，記載下列事項：
一、申請人之姓名、性別、出生年月日、身分證明文件號碼、住居所。
有代理人者，其姓名、性別、出生年月日、身分證明文件號碼、住居所或事務所。
二、主旨。
三、事實、理由及其法令依據。
四、決定機關。
五、決定文號及年、月、日。
六、不服決定之救濟方法、期間及其受理機關。
