Enforcement Rules of the Real Estate Appraiser Act Article 6
In the case that Physical Practicing License of Real Estate Appraiser is missing, destroyed, or stained, the following documentation shall be submitted to the competent authority in a Special Municipality or a County / City to apply for re-issue or renewal of the practicing license.1.Application form.2. For a lost or destroyed physical certificate, a declaration stating that the original certificate has been lost, destroyed, and rendered void shall be submitted. For a damaged physical certificate, the original certificate shall be submitted.3.A copy of Identification Documentation.4.A photograph, two inches in size, of the applicant taken within a year.The valid date of a re-issued or renewed practicing license is the same as that of the original practicing license.

不動產估價師紙本開業證書遺失、滅失或污損，備具下列文件者，得向所在地直轄市或縣（市）主管機關申請補發或換發：

二、紙本證書遺失或滅失者，原證書遺失或滅失作廢之切結聲明；紙本證書污損者，原證書。
三、身分證明文件影本。
四、本人最近一年內二吋半身照片。
依前項補發或換發之開業證書，仍以原開業證書之有效期限為期限。
