Enforcement Rules of the Real Estate Appraiser Act Article 5
In the case that Physical Certificate of Real Estate Appraiser is missing, destroyed, or stained, the following documentation shall be submitted to the competent central authority to apply for re-issue or renewal of the certificate.1.Application form.2. For a lost or destroyed physical certificate, a declaration stating that the original certificate has been lost, destroyed, and rendered void shall be submitted. For a damaged physical certificate, the original certificate shall be submitted.3.A copy of Identification Documentation.

不動產估價師紙本證書遺失、滅失或污損，備具下列文件者，得向中央主管機關申請補發或換發：
一、申請書。
二、紙本證書遺失或滅失者，原證書遺失或滅失作廢之切結聲明；紙本證書污損者，原證書。
三、身分證明文件影本。
