The motion in Paragraph 1, Article 24 of the Act shall be filed in the form of a pleading.A party identifying a verification report as documentary evidence to be introduced shall file a pleading specifically stating the content of the report and attach photocopies of the documentary evidence used, to be submitted to the court, and send a written copy or photocopy of the same directly to the opposing party.

本法第二十四條第一項之聲請，應以書狀為之。
當事人以查證報告書聲明書證者，應於書狀具體記載其內容及添具所用書證之影本，提出於法院，並以繕本或影本直接通知他造。
