Intellectual Property Case Adjudication Rules Article 36
If there are multiple verifiers, they may prepare the verification report to submit to the court jointly, provided they shall do so separately where opinions differ.The verification report shall include the following:(1) the name of the verifier(2) the date, time, location of the verification, names of attendees and their relationships to the verified party(3) where assistance with the verification is rendered by a Technical Examination Officer, the name of such officer, and date, time, location, and content of assistance(4) whether the verified party refuses or obstructs the verification(5) the items, methods, and results obtained of verification(6) other necessary matters

查證人有數人者，得共同製作查證報告書提出於法院。
但意見不同者，應各別為之。
查證報告書，應記載下列各款事項：
一、查證人姓名。
二、實施查證之日期、時間、地點、在場人姓名及其與受查證人之關係。
三、受技術審查官協助實施查證者，該技術審查官姓名、協助之日期、時間、地點及內容。
四、受查證人有無拒絕或妨礙實施查證之情事。
五、實施查證之事項、方法及所得結果。
六、其他必要事項
但意見不同者，應各別為之。
