The Notary Act Article 9

Notaries, when signing in their capacity as notary, shall indicate their title and the court to which they are registered. Civil notaries shall indicate the location of their office.
中文原文 公證法第9條
公證人為職務上簽名時，應記載其職稱及所屬之法院。
民間之公證人並應記載其事務所所在地。
