If the operator certificate is missing, destroyed or there is a change of printed items during the valid period, the applicant shall fill in the replacement application form with related documentary proof attached, and submit the application through the facility operator to the Competent Authority for reissue. The period of effectiveness for the certificate is the same as that for the original certificate.

運轉人員證書於有效期限內遺失、損毀或變更登載事項者，申請人應填具補發申請書，檢附相關證明文件，經由設施經營者向主管機關申請補發，其有效期限至原證書有效期限屆滿為止。
