Administrative Regulations for Radioactive Material and Equipment Capable of Producing Ionizing Radiation and Associated Practice Article 33
Paragraph 33.1For the permit for radioactive material or equipment capable of producing ionizing radiation or the designated items in the registration approved by the competent authority, if there is change of the printed items or loss or damage of the permit, the facility operator shall, within 30 days of the occurrence, fill out an application form and apply to the competent authority for change, reissuance or renewal.Paragraph 33.2The period of effectiveness for the permit is the same as that for the original one.

放射性物質及可發生游離輻射設備許可證或經主管機關同意登記時所指定之項目，有登載事項變更或許可證遺失、損毀者，設施經營者應自事實發生之日起三十日內，填具申請書，向主管機關申請變更、補發或換發。
許可證之有效期間與原證相同。
