Regulations of Marine Dumping Permit Management Article 8
The marine dumping permit document shall record the following items:I. Basic informationA. The name, contact telephone number and address of the applicantsB. The name, adress, contact telephone number and identification document number of the statutory responsible person of the applicants.II. Permit content:A. Name, type, and quantity of the substance to be dumpedB. Dumping areaC. Loading operations, pollution control measures, dumping method, dumping rate and sea transportation itineraryD. Number of times of dumping and dumping frequencyE. Monitoring positions, monitoring items, monitoring frequency, monitoring methods, reporting, record preservation and other binding mattersF. Restrictive terms regarding the dumping periodG. Documents and information on the names of the ships or equipment used for marine dumping; and on the name, identification document number and address, and liability insurance of the statutory responsible personH. Other items designated by the central competent authorityIII. Validity period of the marine dumping permit document and document number

海洋棄置許可文件應記載下列事項：
一、基本資料：
（一）申請單位名稱、聯絡電話及地址。
（二）申請單位負責人姓名、住址、聯絡電話及身分證明文件字號。
二、許可內容：
（一）棄置物質之名稱、種類、數量。
（二）棄置區域。
（三）棄置物質裝載作業、污染防止措施、棄置方法、棄置速率及海上運送航程。
（四）棄置次數及棄置頻率。
（五）監測位置、監測項目、監測頻率、監測方式、申報、紀錄保存其他應遵循事項之規定。
（六）棄置期間之限制條件。
（七）海洋棄置船舶或機具名稱、負責人姓名、身分證明文件字號及住址、責任保險文件資料。
（八）其他經中央主管機關規定之事項。
三、海洋棄置許可文件之有效期間及許可文號。
