If "a separate declaration of agreement", as referred to under paragraph 2 of Article 7 of the PDPA, is to be provided concurrently with other expressions of intent in the same document, the data collector shall, where appropriate on the document, make the data subject aware of the details of such agreement and confirm his/her consent.
