Permits shall record the following items:Ⅰ. The name of the organization.Ⅱ. Name and address of laboratory.Ⅲ. The name of the laboratory manager.Ⅳ. Testing categories, items and methods.Ⅴ. Valid period of time.Ⅵ. Other items designated by the central competent authority.
