Working Standards for Environmental Agent Permit Application and Issuance Article 11
The permit shall register the following items:I. Permit numberII. Name, address and statutory responsible person of the companyIII. Name and address of the manufacturerIV. Type and category of the environmental agentV. Product nameVI. Product expiration dateVII. Performance dataVIII. Dosage form and volumeIX. Composition and contentX. Permit issuance date and expiration dateXI. Other items designated by the central competent authorityWhen finding any written errors or obvious errors that do not comply with the Regulations and relevant provisions on the permit, the central competent authority may make corrections at any time and inform the party to apply for a permit replacement within a prescribed period.

三、製造廠名稱、地址。
四、環境用藥種類及品類。
五、品名。
六、產品有效期限。
七、性能。
八、劑型及內容量。
九、成分及含量。
十、許可證發證日期及有效期間。
十一、其他經中央主管機關指定應登記事項。
中央主管機關發現核發之許可證有誤寫或其他類此未符合本法及相關規定之顯然錯誤者，得隨時更正之，並通知限期換領許可證。
