Permit, Registration and Approval Regulations for Toxic and Concerned Chemical Substances Article 17
Permits, registration documents, and approval documents of toxic and concerned chemical substances shall record the following items:I. Basic information of the handler:A. Name, address, and regulatory number of the handler.B. Name of the statutory responsible person.II. The handling site: name, address, and regulatory number.III. Date of issuance and period of validity.IV. Other relevant notes and annexes.Subparagraph IV of the preceding paragraph shall include the following:I. Name, listed number, series number, and concentration of toxic and concerned Chemical Substances.II. Handling activities, uses, permit number of the licensing agency, and permitted (approved) handling items, etc.III. Original approval date and notes of the handled substances.If the special municipality, county or city competent authority discovers any miswriting, miscalculation, or other obvious errors in the permits, registration documents, or approval documents that do not comply with the Act and relevant regulations, the authority may correct the permits, registration documents, or approval documents at any time and notify the handler.

毒性及關注化學物質之許可證、登記文件及核可文件應記載下列事項：
一、運作人資料：
（一）運作人名稱、地址及管制編號。
（二）負責人姓名。
二、運作場所：名稱、地址及管制編號。
三、發證日期及有效期間。
四、其他註記事項及附件。
前項第四款應登載下列事項：
一、毒性及關注化學物質化學物質名稱、列管編號及序號、濃度。
二、運作行為、使用用途、簽審編號等許（核）可運作事項。
三、運作物質初始核准日期及備註事項。
直轄市、縣（市）主管機關發現許可證、登記文件或核可文件有誤寫、誤算或其他類此未符合本法及相關規定之顯然錯誤者，得隨時更正之，並通知運作人。
