People with Disabilities Rights Protection Act Article 23
Hospitals shall set up service entries for people with disabilities who have difficulty expressing their own needs or other special requirements, to help communication and other health-seeking related services.Hospitals shall provide a discharge planning program for the admitted people with disabilities and the program shall include the following items:1. Home care proposals.2. Rehabilitation proposals.3. Referral services of medical care resources in the community.4. In-home environment improvement proposals.5. Assessment and usage proposals of assistive device appliance.6. Transitional services.7. Reconstruction of daily living service proposals.8. Psychological counseling service proposals.9. Other matters related to discharge plan.The implementation of discharge plan services stated in the preceding paragraph shall be listed as the item of hospital assessment by the central competent authorities.
中文原文 身心障礙者權益保障法第23條
醫院應為身心障礙者設置服務窗口，提供溝通服務或其他有助於就醫之相關服務。
醫院應為住院之身心障礙者提供出院準備計畫；出院準備計畫應包括下列事項：
一、居家照護建議。
二、復健治療建議。
三、社區醫療資源轉介服務。
四、居家環境改善建議。
五、輔具評估及使用建議。
六、轉銜服務。
七、生活重建服務建議。
八、心理諮商服務建議。
九、其他出院準備相關事宜。
前項出院準備計畫之執行，應由中央衛生主管機關列入醫院評鑑。
醫院應為住院之身心障礙者提供出院準備計畫；出院準備計畫應包括下列事項：
